Is there really any evidence for these things...
A woman should not pray in the masjid.
A woman should not pray at home in jam'aa (esp. with only women.)
A woman cannot recite out loud in her prayer.
Also, what is the appropriate way for a woman to make sujood?

Praise be to Allah.

It is important to read books about your way of life to clear up the myths from reality. We recommend you purchase a set of Sahih Al Bukhari and Sahih Al Muslim and read about your way of life.

In regards to Praying, there is no difference in the way a man prays or a woman. Women make sujood the same way as men do and that is by placing your head on the floor between your hands and with your elbows out to the side. while on your knees. All positions of salat are performed the same way for men and women. Allah does not discriminate.

In regards to a woman praying at a mosque, women are not FORBIDDEN to pray at the mosque nor are they forbidden to go to the Mosque. However, a woman gets MORE BLESSINGS PRAYING IN HER HOME THAN AT THE MOSQUE:

Ibn 'Umar reports that the Prophet sallallahu alehi wasallam said: "Do not prevent the women from going to
the mosques, although their houses are better for them." [Related by Ahmad and Abu Dawud].


Ahmad and at-Tabarani record that Umm Humaid as-Sa'diyah came to the Messenger of Allah and said: "O Messenger of Allah, I love to pray with you." The Prophet said: "I am aware of that, but your salah in your residence is better for you than your salah in your people's mosque. And your
salah in your people's mosque is better than your salah in the [larger] congregational Mosque."

In regards to a women praying in congregation and reciting Quran aloud there is nothing in Islam that prevents or prohibits a woman from any of this in fact this is how women should pray:

Sa'id and Abu Hurairah both report that the Prophet
sallallahu alehi wasallam said: "Whoever gets up during the
night and wakes up his spouse and they pray two rak'at
together, they both will be recorded among those (men and
women) who remember Allah much." [This is related by Abu
Dawud].

It was narrated from Tameemah bint Saleemah that ‘Aa’ishah the Mother of the Believers (may Allaah be pleased with her) led women in Maghrib prayer, and that she stood in the middle of them and recited Qur’aan aloud.

It was also narrated that Um Salamah the Mother of the Believers (may Allaah be pleased with her) used to lead women in prayer in Ramadaan, and that she stood with them in the row.

And it was narrated that Ibn ‘Umar used to tell one of his slave women to lead his womenfolk in prayer in Ramadaan.”
