I have had this problem with three different dogs. One got a series of staph lysate injections as a puppy and never had a reoccurance. One got two series of the staph injections and more antibiotics than I could count and it did nothing. He had his problem for two years and then we moved and it cleared up never to return for the rest of his life. It was allergy related - something in the region where we were living. He also had constant yeast infections in his right ear which never cleared up. Right now our little Crested has a staph infection caused by a sensitivity to benzoyl peroxide shampoo (one of the medicated ones) and she has had two months of antibiotics and it is not gone yet. It caused severe drying of her skin which also helped the staph to spread. Staph causes itching all by itself so do be aware of that. However, with his history of ear infections, it does sound like your boy is allergy prone. Does your vet want to give him general allergy shots or shots of staph to boost his immunity to it and hopefully resolve it? I know that the costs can be crippling so before you start, get a very good idea whether the vet expects there to be significant improvement or is just trying another avenue. Discuss all pros and cons over very carefully with your vet and it is not inappropriate to bring up cost as a factor as you must be able to take care of other problems that your dog has as well since he is an epileptic. I would certainly try a diet change if you have not done so. There are other foods such as Wellness that have only two ingredients that many dogs do well or you might try to home cook.
This is a difficult problem as I too well know for some dogs. You may want to consult a holistic vet in addition to your regular vets to see if he/she could add anything that might help strengthen his immune system and help him fight the staph.
I have great sympathy for you since I am currently in the same boat! If you would like a second opinion, you might see if there is a vet school nearby and consult one of their dermatologists or just another dermatologist in a different practice. Sometimes vets do treat things from a different perspective.
Sometimes dogs do remain on antibiotics for long term treatment which is not too healthy for them in general but there may be no choice. In short, this is a frustrating problem with no real short good answer! I hope that it will clear up for your boy though and he will improve.
