Yes, we should (and will) find out how it happened, but this is complex and dangerous stuff we are talking about. In the words of one of the tech-support guys at work, "sometimes complicated things just break...". There may not ever be a good answer that neatly points to one, two or a dozen people at fault.

You make a good point about the technology though...we are send the equivalent of a Dodge Aspen into space, all be it with new electronics (but still with that 225 CI slant six engine...as in the shuttle engines being of a 1970's cutting edge of technology).

If we are going to be in space...AND WE SHOULD...maybe this is a wake-up call that this needs to be done RIGHT. We 'piss' money away all the time in the government on things that add nothing to the greater understanding of mankind...it's about time fully and rightfully fund these endeavors. They speak to the best and the highest ambitions of human kind.

Lastly, this truly is a terrible and tragic event. But, with that said, these heros died doing what they loved to do...
