I can see a few options.
Option One: Petrosian finds a more compliant pet during his takeover bid in New Regime, and Operations *does* get poisoned. Madeline is most likely transferred out of Section entirely.
Option Two: Operations survives the events of New Regime, but Madeline is killed during the events of the very next episode, Mandatory Refusal. Her replacement is most likely someone from outside Section One, whose loyalty to Operations might be questionable. Internal politics therefore takes out Operations sooner rather than later -- possibly via the attack by Adrian (although Adrian's plans might also have been set back by Nikita's death).
