 I find the Proms usage especially odd as (to the best of my knowledge) Rachmaninov himself chose to be known as that when he lived in the USA. A comparable situation is that of Händel, who chose to be known as "Handel" when he moved over here.

I don't think the 'ch' in "Vasilyevich" derives from French usage. "Vasilevitch" seems more common in French, as does "Khatchaturian".
