Good questions (and answers). I wonder if she wishes she had started earlier.

She mentions the euphoric feeling and feeling better when she craves and lights up. The way nicotine works is really fascinating to me. Remember the scene in The Matrix when Cypher is back in the Matrix and eating steak with Agent Smith? he says something along the lines of "I know the the Matrix is making me THINK that this steak is juicy and delicious. Ignorance is bliss." Smoking does not really make you feel good it is not smoking that feels bad. Yet you do not care. It is an interesting construct

I bet her lungs are not different at all. That takes many years and even then not everybody has lungs that are black although that certainly happens

How has her friend that got her to try smoking reacted to all of this?
