THis is one of the dogs from the 70 in Texas, correct? If so, then it's my understanding that these dogs were severely neglected, they had little to no human contact and were left to run outside with other Cresties and Chis...in other words, they are basical feral.
Winning this dog over is going to take lots of patience, love and time. There is no overnite fix, he has got to learn to trust you (and all humans) on his own timetable. My best advice to you is to let him do things at his pace. If he wants to stay in his crate, let him...NEVER reach in the crate to get him out. Take the top off so you can get to him from above. If you push him too far too fast, you totally erase any steps forward he may have taken. Give him a bowl of food & water in his crate and let him eat it at his choosing (normally I don't advocate free feeding, but this is a case where you need to win him over before anything else...and please no more ham, it is far too salty - if you want to bribe him, boil up some chicken or hamburger).
I would also suggest that you purchase some homepathic anxiety drops (I swear by Anxiety made by Homeopets). Simply place the drops in his water bowl to keep him on an even keel. If you see little to no relaxtion with those, I would suggest contacting your vet and asking for a Rx of Xanax. Again, normally I do not advocate using these types of meds accept as a last option, but his anxiety and stress can lead to over health issues so helping him medically to alleviate some of his stress level is important.
As for the coughing/gagging, that could be from stress or he could have a touch of kennel cough. If it continues, he should be seen by a vet.
Hang in there, don't rush him and in time, you'll have the most devoted and loving little dog you could ever wish for.
