If the foot does not touch the ball, how has the ball been played??

Was the ball rolled back by the hand? (This apparently gives better control over the ball than using the foot, and coaches would like it as it serves the dummy-half better)
Did the player step over the ball? (Frequently done in an attempt for speed)
Was the ball placed in front of the foremost foot? (Probably not)

Are these actions accidental? No, they are the result of coaching and trying to gain an advantage.

Stick with the penalty!
