What a great opportunity to encourage player creativity -- where feats pushed old 1st edition hack and slash to a tactical level -- this method goes even further.

I can now describe my actions fully and see the direct result of throwing sand in someone's face or knocking the wind out of them or making an especially rude sound!

The really difficult aspect of this is that I see no way for it to avoid slowing down combat -- and combat can be slow enough as it is! Secondly, I can see this system being horribly abused -- more than likely people will STOP beig creative and fall into the same set of manuvers that worked so well the last time they used them.

I think, ideally, these sorts of rules should be kept in the back pocket for very special inspired occasions, like when Osthryd broke through the stage in the Cauldron Campaign. Under Conan rules, perhaps a fate point could be spent to pull off a very special attack that could use these rules.

While the DM should take an active role in making combat come alive, I think the players share an equal responsability. It would be fun to describe your actions as you fight instead of just, "I hit and deal 8 pts of damage".
